If you are unable to print the form, please provide the required information on a sheet of paper and send with your payment to the nearest PSC office. Please be sure to provide a photograph, full name, birthdate and birthplace for yourself and for each family member.

Application Fee

Entries for the DV program are only accepted electronically. All the applicant's information, including the photographs, must be uploaded onto the US Department of State website. The website becomes overloaded and often shuts down as the deadline for the program approaches - many applicants wait until the last moment to file their entries.

Because of the constant shut downs of the Department of State website, it takes many attempts, and more time, to successfully file our clients' entries. For this reason, our fee increases the later you apply.

If you are married, we encourage you to file a double application to increase your chances of selection.
