ENTRY PROCEDURE

Complete the DV-2007 entry form in full and send with payment to the nearest PSC office. You may only submit one application. However, a husband and wife may each make a separate entry and thereby double the chances of selection.

The registration period for DV-2007 is between, October 5, 2005 December 4, 2005 . All entries must be received at a PSC office before 5:00pm December 2, 2005 in order to meet the registration deadline.

As of DV-2004, the US Dept of State retains all the entries, with names and addresses, and makes them available to other government agencies. PSC lists the Lexington office address on each client's entry form. Your address will not be disclosed to anyone without your permission.
